How to Use This Affidavit

Step 1: Fill in all fields above. Write your statement as factual paragraphs — what you personally saw, heard, or did. Include dates, times, and locations.
Step 2: Print the affidavit. Do NOT sign it at home — you must sign in front of a notary public.
Step 3: Find a notary — most banks offer free notary services, or search "notary near me." Bring photo ID. The notary will witness your signature and stamp the document.
Step 4: Attach any exhibits (text screenshots, photos, documents) referenced in your affidavit. Label them Exhibit A, B, C, etc.
Step 5: File with the court clerk's office or via eFileMA. Serve a copy on the other parent. Keep copies for yourself.
⚠️ Rules: Be truthful — false statements are perjury (a crime). Stick to facts you personally witnessed. Don't include hearsay ("someone told me..."). Don't include legal arguments — those go in your motion, not your affidavit. Keep it concise and organized.
